Symphony Care Management provides expert guidance for older adults and their families facing new or ongoing health challenges, including complex medical situations such as Alzheimer’s and other forms of dementia.
Central to our role is building and coordinating a specialized team of people and services to support clients, each in a different way, to achieve their personal goals. We bring a holistic approach to Care Management and believe in building a strong foundation of trust through open and honest communication.
An Ally and An Advocate
Everyone wants to age gracefully and with dignity, and this often includes the desire to maintain as much independence as possible for as long as possible while ensuring health and safety needs are met. As your ally and your advocate, we work hard to help you achieve your goals.
At Symphony Care Management, we have a deep knowledge of all available resources and can recommend the most suitable options based on your unique circumstances, including in-home supports, residential options, or other levels of care. If your loved one requires help with cooking, cleaning, or taking medications, we are here to coordinate and manage those services to make sure they run smoothly and effectively. Our trained eyes and ears will watch for signs that a given level of services needs to change and we are there to guide you through next steps. We are available to take clients to appointments and we communicate regularly with medical professionals about our clients’ physical and/or mental health challenges to help determine which services are needed, and how best to provide them in accordance with our clients’ values and wishes.
Family members often struggle to navigate uncharted territory when it becomes clear their loved one requires additional supports in their day-to-day life. Through our compassionate Caregiver Coaching and Alzheimer’s Caregiver Coaching services, we can help families understand the issues, solve internal conflicts or disagreements, and ease the stress of new and unfamiliar challenges.
Advocacy on your behalf is a core strength at Symphony Care Management. We effectively promote your wishes across the spectrum of care and supports you are receiving. We know what’s possible and will make sure you get the level of care you desire and deserve.
Developing and Implementing a Symphony Care Management Plan
The first step in developing a customized Care Management Plan is an in-depth Assessment of our clients’ current physical, cognitive, psychological, social, and legal needs by an experienced Certified Care Manager. This includes interviews with our client, their current caregivers and medical providers (if desired), an assessment of their physical environment, an overview of available resources, a review of advanced directives, and a discussion of their personal values and wishes.
Based on this Assessment, we will provide a comprehensive Recommendations Report detailing our observations and recommendations which will, in turn, form the basis of your personalized Symphony Care Management Plan should you choose to continue working with us.
When clients join the Symphony Care Management family, we continually reassess their changing needs, problem solve, respond to crisis, coach the care team and facilitate communication between team members and families to ensure everything works like a well-oiled machine.
