Transitions for adults with a diagnosis of cognitive impairment are even more difficult than when there are strictly physical limitations. Families struggle to accept their loved one’s inability to see the reality of their needs. Behaviors and changes in personality that come with Alzheimer’s disease and related disorders can be disturbing. At the same time, caregivers often neglect their own emotional and physical needs. Caregiver coaching requires an assessment of the client’s history and the family system that surrounds the older adult and is a valuable benefit to those struggling to cope. Your Symphony Care Manager will:
Educate the primary caregiver and other family members about best practices for caring for a loved one with Alzheimer’s or a related dementia. It is not uncommon that caregivers don’t fully understand this disease.
Ensure the proper diagnosis. It is critical that clients have a full work up from a specialist so that any reversible reasons for the symptoms of dementia have been ruled out. Understanding the client’s strengths and weaknesses cognitively will assist in ensuring appropriate medical care.
Personalize strategies to:
1. Enhance the client and caregiver’s daily quality of life
2. Reduce safety risks to the client and caregiver
3. Prepare our client for successful transitions.
Provide emotional support and:
1. Mediate family disputes or disagreements with a personalized plan
2. Assist caregivers in taking time off to regenerate their energy and health.
Build a team. We will pull together the right vetted partners and coordinate care.
Provide short- and long-term planning through Symphony Care Management.
